- Business Expo Application - all exhibitors are required to submit a signed application form.
- Butter Braid Coffee Cake Order Form - available annually at the Parish Business Expo.
- The Caring Place/The New Faces Partnership Expo Applicaton - all exhibitors are required to submit a signed application form.
- Cause MSA Form - Used by Ministry for Social Action to distribute parish's annual outreach tithing; once the form is completed it can be attached to an eMail to Parish Manager, Dr. Honora Norton, or dropped off at the parish office.

- CM-Religious Education Forms: A form (serves as family folder with family information, registration choices and contact information) can be picked up at the CM Office: Sunday 7:30 – 1:00 pm, M-TH 9:00 – 4:00 pm or call 480-998-1665.
- Commitment Form - want to commit some of your time, talent and/or treasure to our parish. Use this form to show your interest. Refer to Ministry Booklet and Homepage for list of ministries.
- CTODP Contribution Form: Redirect your State of Arizona taxes to CTODP, you will help create tuition assistance scholarships for our parish school and/or other school with low income families that desire but often cannot afford a Catholic education.
- 2011 Emmaus Retreat with Persons with Intellectual Disabilities Registration Form - The next Retreat with Persons with Intellectural Disabilities is scheduled for August 19, 20 & 21, 2011 at the Living Water Retreat Center. To register, complete the form on the computer by typing the information directly into the form, then you will print it out and send it to the parish (c/o Emmaus Retreat, Blessed Sacrament Catholic Church, 11300 N 64th St, Scottsdale, AZ 85253) or eMail it to CM office. - Employment Application Form: to completed as part of interview process for open staff positions.
- Engaged Couples Registration Form and Marriage Preparation Seminar Dates (see form).
- Event Exhibitor Form - required for each Exhibitor/Business displaying and/or promoting their products and/or serves at a parish sponsored event. This is exclusive of Parish Business Expo - to access forms for the Expo go to Business Expo Webpage.
- Expense Request Form (i.e. purchase order)
- Fundraiser/Event Request Form - required for each parish fundraiser and/or special event - i.e. sale of goods/services, dinners, auctions, raffles, golf outing, dances, anniversary parties, freewill gifts for concerts, workshops, speakers, etc. It should be submitted to Parish Office Manager. Pastor's and Parish Manager's approval is required before fundraiser and/or special event can promoted and/or initiated. There is a parish policy regarding raffles - per Arizona law - it must be followed to avoid liability and/or fines from the State of Arizona. There is a parish/diocesan/insurer policy regarding alcohol - per Arizona law that must be followed to avoid event shutdown, fines and/or prosecution. As an added note, event/fundraiser chairs must contact the Parish Office Manager to discuss required procurement of liability insurance and/or providing insurer certificate of liability for the event or fundraiser, as well as, discuss procurement/payment for required maintenance staff services needed for setup, takedown and during special events or fundraisers.
- Golf Outing Registration Form (2012) and 2012 Golf Outing Poster.
- Habitiat for Humanity Volunteer Form: Each volunteer must complete, the a H4H Volunteer Information & Waiver Form and return it to the parish office - attention: Dr. Honora Norton, Parish Manager. Deadlines vary per H4H project.
- Incident (Accident) Report - to be used in cases of any accident, property damage, theft, acts of violence, emergency situation (EMT called, fainting, falling...) and the like. Must be completed and on file at the parish for submission to parish's insurance carrier. This form is available in the church sacristy, in the parish office and on this website.
- Just Coffee Order Form - "Fair-trade-Just Coffee" sales is sponsored by Just faith Small Christian Community. This summer, order Just Coffee products online at www.justcoffee.org or complete this order form. Coffee may be picked up in the parish office two weeks after the order is placed.
- Knights of Columbus High School and College Scholarship Applications

- Ministry Booklet - lists parish ministries and contains Ministry recommit and/or signup form.
- Ministry Definition Form - used to describe how a parish ministry supports parish's 4-Pillar Stewardship Process.
- New Parishioner Registration Form - new to the parish? Please fill out our registration form and drop off at the parish directory so we can make sure to include you with all parish communications.
- Scrip Order Form - Gift Cards (Scrip) are available at NO EXTRA cost to you and are available after the Saturday evening Mass and the Sunday morning Masses at the Women's Club table in the courtyard, as well, as during business hours in the parish office. See this form list of cards available.
- Volunteer/Event Form Hold Harmless Form - required for any offsite event and/or special event, fundraiser or outreach service project - to ensure liability issues are addressed. Parish ministry volunteers can only provide services of ministry (i.e. Ministers of Care (MOC) can only bring Eucharist to person; they can not pick-up Rx, groceries, be caretaker or even change a light bulb - the MOC is not allowed to do any out-of-ministry tasks. If MOC wants to assist, then they must be go through the
DUET Partners in Health & Aging Volunteer Training - see Caring Place.
- Work Order - form required to decribe meeting/event setup requirements and/or instructions: i.e. include drawing of table/chair setup (including numbers requires), A/V requirements, define timing, etc. Parish maintenance staff will use this form to prepare reserved meeting places for your event and/or meeting. The form should be sent to the maintenance staff at least one week prior to event.
